Eligibility Requirements
- Members who have a minimum of 280 hours are automatically enrolled in the health plan and are eligible for coverage. Hours must be paid working hours under a Local 873 contract to establish initial eligibility for benefits.
The vision benefit is available to members and dependent’s every 24 months.
- The benefit is paid by Canada Life.
- You may submit a claim for yourself and any dependent (spouse/children) who are on your benefit plan.
- The receipt must not be more than 12 months old.
- The maximum benefit payable is $650 every 2 years per person and may be used for prescription eyeglasses, contact lenses, and eye exams.
- If you have additional costs above the $650, you may submit the remainder of your receipt to Canada Life for reimbursement from your Health Spending Visa account provided that: You are enrolled in the Health & Dental plan AND You still have funds available to use.
