Accidental Death & Dismemberment

Policy# 10453601

If you are eligible for Life Insurance coverage you will also receive AD & D coverage of $100,000 for members under age 70 and for those age 70-75 an amount of $50,000. Members who are both Active and Retired (Chubb – Critical Illness and AD&D – ) are eligible for Accidental Death and Dismemberment coverage.

The benefit amounts and age requirements are the same as for your Life Insurance coverage and this policy is underwritten by CHUBB Life Insurance Company of Canada (Formerly ACE INA Insurance Group). All your benefits remain the same; this is only a name change.  The benefit booklets will be updated to reflect the name change shortly.

Beneficiary

It is understood that the beneficiary designation made on your Group Life Insurance Policy with Canada Life will also be the beneficiary under your AD&D policy. If a beneficiary is not named, your Accidental Death benefits will be paid to your estate.  In the event of a non-death claim, all other indemnities of the policy will be payable to you.

You can change your beneficiary at any time, where permitted by law.  CHUBB Life and the Trust assume no responsibility for the validity of such designation or change of beneficiary.  Please make sure you review your beneficiaries at least once a year to ensure it reflects your current situations and intentions.

Bereavement Benefit

When injuries covered by the policy result in loss of life of an Insured Person within 365 days from the date of the accident, CHUBB Life Insurance will pay the reasonable and necessary expenses actually incurred by your spouse and any dependent children for up to 6 sessions of grief counseling, by a “Professional Counsellor”, subject to a maximum of $1,000.   A “Professional Counsellor” means a therapist or counselor who is licensed, registered, or certified to provide such treatment.
