As part of the Hour Bank – health and dental plan you will have a Global Medical Assistance card (GMA). You should carry this card with you whenever you are traveling outside your home province.

If you or a family member is covered under your plan, are hospitalized, contact Global Medical Assistance (GMA) within 24 hours of the emergency. The phone number is on the back of your GMA card. (Failure to contact GMA within 24 hours may compromise your ability to have your claim managed and to have payment coordinated with your provincial health care.)

Plan members who find themselves in need of medical assistance while travelling will have new phone numbers to use beginning January 1, 2014.   All phone numbers that are currently in place will also continue to work so there is no need to replace the GMA card that you have currently.

Normally, GMA will coordinate direct payment of the service with the provider. If you must pay for a covered medical expense yourself:

Keep all medical receipt(s).

Obtain a fully itemized bill for any hospital treatment.

Complete and submit the two claim forms below within 30 days of your return home.

Send your completed forms to Canada Life along with the original receipts and itemized bills. (Be sure to keep a copy for your own records.) Original receipts will not be returned. You can submit your claim online through your GroupNet profile or mail your form to

Approved claims are paid in Canadian dollars based on the exchange rate in effect when the expense was incurred.
